Clinical evaluation of the use of statins in diabetic dyslipidemia

Abstract
For diabetic patients, cardiovascular disease (CVD) is known to be the most frequent
cause of mortality. Due to the increased risk of CVD, the appropriate management of dyslipidemia by using statins is very important for maintaining low density lipoprotein cholesterol (LDL-C) below the target level. This study aims to evaluate the appropriate use of statins for controlling dyslipidemia in patients with diabetes.
This study analyzes lipid profiles of the diabetic patients (age> 40 years), who started using
statins for the first time in the first half of 2010 in one of our facilities. The criteria for evaluation
to begin statin therapy included a high LDL-C level of over 100 mg/dL the target level of treatment was LDL-C level <100 mg/dL those who do not suffer CVD or LDL-C level <70 mg/dL
for those who have CVD. The evaluation criteria for target levels have been decided by referring to updated NCEP ATC 3 guidelines and ADA & ACC guidelines.
As a result, we learned that 97% of our patients started using statins properly. However, only
37.9% percent of patients reached their target LDL-C level. This was lower than 47.4%, which
has been indicated by other case studies conducted by KDA (Korean Diabetic Association). In
addition, no patients took the LDL-C level test as recommended by the standard guideline. Thisis probably because of long-term prescription intervals.
Therefore, patients who are taking statins for the first time are required to visit the hospital
more frequently for the follow-up prescription. In addition, patients should be made aware of the necessity for periodic follow-up. In addition, pharmacists are expected to confirm medication compliance to control LDL-C levels for diabetic patients.
